wrist/hand ULNAR COLLATERAL INJURY OF THUMB
DEFINITION:
- Also known as “Skier’s thumb” or “Gamekeepers thumb”.
- An injury to the ulnar collateral ligament of the thumb metacarpophalangeal (MP) joint usually at its attachment to the proximal phalanx. (See illustration).
MECHANISM OF INJURY / HOW IT OCCURS:
- Frequently a result of straining the thumb against a ski pole.
- Also seen in football, basketball, volleyball, wrestling, and from a fall.
SYMPTOMS:
- Individuals complain of sudden pain to the inner (ulnar) portion of the thumb MP joint.
- Weakness with pinch activities.
DIAGNOSIS:
- Physical examination will reveal swelling and tenderness over the inner MP joint.
- Stress to the ligament may reveal laxity indicative of at least a partial tear.
- Radiographic studies to evaluate for possible fracture or dislocation.
TREATMENT/REHABILITATION:
- Initial treatment entails splinting/casting for a sprain or partial tear.
- Complete ruptures often require surgical intervention.
