wrist/hand ULNAR COLLATERAL INJURY OF THUMB

DEFINITION:

  • Also known as “Skier’s thumb” or “Gamekeepers thumb”.
  • An injury to the ulnar collateral ligament of the thumb metacarpophalangeal (MP) joint usually at its attachment to the proximal phalanx. (See illustration).

MECHANISM OF INJURY / HOW IT OCCURS:

  • Frequently a result of straining the thumb against a ski pole.
  • Also seen in football, basketball, volleyball, wrestling, and from a fall.

SYMPTOMS:

  • Individuals complain of sudden pain to the inner (ulnar) portion of the thumb MP joint.
  • Weakness with pinch activities.

DIAGNOSIS:

  • Physical examination will reveal swelling and tenderness over the inner MP joint.
  • Stress to the ligament may reveal laxity indicative of at least a partial tear.
  • Radiographic studies to evaluate for possible fracture or dislocation.

TREATMENT/REHABILITATION:

  • Initial treatment entails splinting/casting for a sprain or partial tear.
  • Complete ruptures often require surgical intervention.
